Remote Patient Monitoring
& Chronic Care Management
We use remote patient monitoring (RPM) to capture data from blood pressure machines, blood sugar monitors, pulse oximeters (for oxygen levels in the blood), scales, and other devices and wearables that also connect through cellular towers and bluetooth, overcoming limitations due to broadband internet/wifi or smartphones. These RPM devices help combat the lack of broadband internet/wi fi and smartphones in neighborhoods across the country as an emerging social determinant of health. We stand with advocates for health equity.

Our progams also collect data on the following and use these to enhance patient care and well-being: blood pressure, heart rate, continuous electrocardiography, continuous glucose monitoring, oxygen levels, body temperature, body fat percentage, activity levels, health status, diet, lifestyle digital journaling, goals, mindfulness, action plans, surveys and questionnaires, pollen count, and air quality, among other features.
